Effective and last updated:
This policy explains how Studio 84 in Milwaukee, Wisconsin handles personal information through studio84.ink, including consultation booking and tattoo consent forms, and through our private staff workspace. It also explains how to ask us about the information you provide.
1. Information we collect
- Consultation bookings: your name, email address, selected artist, appointment date and time, booking status, the terms version you agree to and the time that agreement is recorded, and any phone number, text-message preference, tattoo ideas, or notes you provide.
- Reference photos: images you choose to upload, along with file details such as the file type, size, and upload time. These are attached to your consultation for private review, rather than published in an artist’s gallery.
- Tattoo consent records: your name, date of birth, age, mailing address, selected artist, procedure date, answers about alcohol or mind-altering substances and skin conditions, any details you provide, your consent, typed signature, and submission time. Your artist’s approval and any later procedure notes or adverse effects may also become part of the record. These records contain sensitive personal and health-related information.
- Technical information: our hosting and security services process information such as IP addresses, browser and device information, requested pages, and request times to deliver and protect the website. The application also uses a hashed IP-based identifier to limit repeated form submissions.
- Staff information: approved staff email addresses, sign-in identifiers, artist assignments, account status, and records of actions such as consent approvals.
Please limit notes and uploads to information relevant to your visit. Do not upload identification documents, payment-card details, or unrelated medical records.
2. How we use it
We use this information to arrange and manage consultations, let your artist review your ideas, assess whether a tattoo procedure can proceed, document consent and the procedure, respond to questions, maintain required records, and protect the website against misuse. Staff information is used to control access and record who performed an action.
Consultations are free, and the website does not currently collect payment-card information or process online payments. Automated customer email confirmations and text messaging are not currently active. If appointment texting becomes available, the booking form will explain the optional choice before you request messages.
5. How long we keep it
Tattoo consent and procedure records are retained for at least two years after the procedure, consistent with Milwaukee’s tattoo recordkeeping guidance. A longer period may be necessary for legal obligations, disputes, or related recordkeeping.
Booking details and reference photos remain in our records after an appointment or cancellation unless they are separately removed. The website does not currently apply an automatic deletion schedule to booking, photo, or consent records. Canceling a consultation does not delete its record. Contact us to request removal; we will consider the request against any continuing need or obligation to retain the information.
6. Your choices and requests
You can browse portfolios without submitting a form. Booking notes and reference photos are optional. The forms identify the information needed to complete a booking or consent record; if you prefer not to provide it online, speak with us in person about your options.
You may ask to access, correct, or delete information you have provided by writing to the address below or speaking with the studio owner. We may need to verify your identity before acting on a request. Some records must be retained, and corrections to signed consent records may need to be documented as an additional note rather than replacing the original. Any additional rights under applicable law remain available to you.
7. Keeping information secure
We use HTTPS, staff authentication, access restrictions by artist, and private file storage for consultation uploads. No website or storage system can guarantee complete security. Avoid sharing your private booking link, and contact the studio if you believe someone has accessed your information without permission.
8. Age requirements
Our tattoo services and consent form are for adults aged 18 or older. This website is not directed to children under 13, and we do not knowingly collect their personal information. If you believe a child has submitted information, contact the studio so we can review and address it.
9. Policy updates
We may update this policy when our services or information practices change. The latest version and its effective date will appear on this page. Where required, we will provide additional notice or obtain consent before making a change.
